Every zone, explained
Brain and mood
- Low T symptoms
- Low mood, brain fog and low drive. Easy to blame on work stress or night shifts, which is why a blood test matters.
- What TRT changes
- In men with confirmed low T, mood, energy and motivation often improve. Effects are modest in men whose levels were only borderline.
- What doctors monitor
- No blood test for this one. Doctors track how you feel against your symptoms at the start.
Throat and sleep
- Low T symptoms
- Poor, unrefreshing sleep. Short or broken sleep also lowers testosterone, so the two feed each other.
- What TRT changes
- TRT can make obstructive sleep apnea worse, especially in heavier men who snore.
- What doctors monitor
- A sleep study if you snore loudly, stop breathing at night or feel sleepy all day.
- Tests
- Sleep study (if symptomatic)
Hair and skin
- Low T symptoms
- Thinning body and facial hair, and needing to shave less often.
- What TRT changes
- Acne and oily skin are common early on. Men prone to male pattern baldness may see scalp hair loss speed up.
- What doctors monitor
- No routine lab. Tell your doctor early about acne or faster hair shedding.
Chest
- Low T symptoms
- Some men develop breast tissue (gynecomastia), often linked to a shift in the testosterone to estrogen balance.
- What TRT changes
- Part of any testosterone is converted to estradiol, so estrogen rises on treatment too.
- What doctors monitor
- Estradiol at baseline, and again if breast tenderness or swelling appears.
- Tests
- Estradiol₱2,700
Heart and blood
- Low T symptoms
- Mild anemia, which adds to tiredness.
- What TRT changes
- Raises red blood cell count (hematocrit), and can affect blood pressure and cholesterol.
- What doctors monitor
- Hematocrit is the most watched safety number on treatment. Lipids are checked at the start.
- Tests
- CBC₱495Lipid profile₱1,575
Belly and metabolism
- Low T symptoms
- Fat that settles around the middle, and insulin resistance. Belly fat itself lowers testosterone.
- What TRT changes
- In men with low T, fat mass tends to fall and insulin sensitivity can improve. Weight loss works in the other direction too.
- What doctors monitor
- Blood sugar markers when diabetes or prediabetes is suspected.
- Tests
- HbA1cFasting blood sugar
Muscle
- Low T symptoms
- Losing muscle and strength without a change in training.
- What TRT changes
- Lean mass and strength usually increase, more so with resistance training.
- What doctors monitor
- No routine lab. Progress is judged by strength and body composition.
Bone
- Low T symptoms
- Lower bone density, which raises fracture risk with age.
- What TRT changes
- Bone density tends to improve over the first year or two in men with low T.
- What doctors monitor
- A DEXA bone scan for older men or anyone with a fracture history.
- Tests
- DEXA scan (older men)
Prostate
- Low T symptoms
- Nothing typical here.
- What TRT changes
- Testosterone supports prostate growth, so doctors watch it over time.
- What doctors monitor
- PSA before starting for men over 40, then again during monitoring.
- Tests
- PSA₱1,990
Sexual function
- Low T symptoms
- Low libido, fewer morning erections and weaker erections. These are the symptoms most specific to low T.
- What TRT changes
- Libido usually improves. Erections improve in some men, but many with ED have other causes too.
- What doctors monitor
- No lab for this. Doctors ask about libido and erections at each review.
Testes and fertility
- Low T symptoms
- Smaller or softer testes in some men.
- What TRT changes
- TRT switches off the signal for sperm production, so it works like a contraceptive and can cause infertility.
- What doctors monitor
- LH and FSH show where the problem starts. A semen analysis for men planning children.
- Tests
- LH₱1,760FSH₱1,800Semen analysis
What it costs here
Tired, low drive, gaining weight around the middle? It might be testosterone, and it might not. A morning blood test settles it, and the numbers are less scary than most men expect. The direct free T test is the expensive one at ₱12,600, and there is a cheaper way around it.
